Fibula anatomisinin değerlendirilmesi ve klinik önemi

Özet

Amaç: Fibula’nın proksimal, corpus ve distal segmentlerindeki yapıları kapsamlı bir şekilde incele- mek, klinik açıdan anlamlı boyutlarını değerlendirmektir. Yöntemler: Araştırma 23 fibula (13 sol, 10 sağ) üzerinde gerçekleştirilmiştir. Morfometrik parametre- ler gonyometre, dijital kumpas ve mezura kullanılarak ölçülmüştür. Morfolojik parametreler, iki araş- tırmacı tarafından bağımsız olarak yapılan gözlemlerin karşılaştırılmasıyla değerlendirilmiştir. Bulgular: Ortalama fibula uzunluğu 340,49±22,06 mm ve ortalama foramen nutricium mesafesi 165,53± 33,04 mm bulundu. Facies articularis capitis fibulae şekli, en sık oval formunda (%34,8); fossa malleoli lateralis şekli ise en sık konkav formunda (%91,3) gözlendi. Toplam 25 adet foramen nutricium (19 tek, 3 çift) bulundu, bunların tümünün fibula’nın 1/3 orta bölümünde konumlandığı saptandı. Foramen nutricium’un gözlendiği fibula yüzleri ve kenar bilgilerine bakıldığında en sık fa- cies posterior üzerinde olduğu görüldü. Örneklerin %26,1’inde crista medialis’in facies posterior’da bulunmadığı gözlendi. Foramen nutricium yönünün en fazla oranda yukarı yönelimli (%72) olduğu gözlendi. Sonuç: Tüm fibula segmentlerindeki yapılar, önceki çalışmalara kıyasla daha geniş bir parametre yel- pazesi kullanılarak toplu ve eş zamanlı olarak incelenmiştir. Fibula’ya dair veriler, yaralanma mekaniz- masını, kemik parçası analizlerini, fibula rekonstrüksiyonunu ve foramen nutricium’a zarar gelmesini önleyen cerrahi prosedürleri belirlemek için kullanılabilir.
Aim: This study aimed to examine in detail the structures in the proximal, corpus, and distal seg- ments of the fibula and to assess their clinically relevant dimensions. Methods: The study was carried out on 23 fibulae (13 left, 10 right). Morphometric parameters were measured using a goniometer, digital caliper, and measuring tape. Morphological parameters were evaluated by comparing the independent observations of two researchers. Results: The mean fibular length was 340.49±22.06 mm, and the mean distance to the nutrient foramen was 165.53±33.04 mm. The shape of the articular facet of the fibular head was most fre- quently oval (34.8%), whereas the malleolar fossa most commonly had a concave form (91.3%). A total of 25 nutrient foramina (19 single, 3 double) were identified, and all of them were located in the middle one-third of the fibula. When the surfaces and borders of the fibula on which the nutrient foramen was observed were examined, it was found most frequently on the posterior surface. In 26.1% of the specimens, the medial crest was not present on the posterior surface. The direction of the nutrient foramen was most often directed upwards (proximally) (72%). Conclusion: The structures in all fibular segments were examined collectively and simultaneously, using a wider range of parameters than in previous studies. Data relating to the fibula may be used to clarify injury mechanisms, to perform bone fragment analyses, to plan fibular reconstruction, and to determine surgical procedures that avoid damage to the nutrient foramen.
