THE USE OF STEM CELLS AND STEM CELL-DERIVED AGENTS IN PERIODONTAL THERAPY: CLINICAL STUDIES AND FUTURE PERSPECTIVES

Özet
Abstract: Periodontitis is a chronic inflammatory disease that leads to the destruction of the periodontal supporting tissues, and is one of the most common causes of tooth loss worldwide. Although mechanical and surgical therapies are used in its treatment, they are insufficient in achieving complete regeneration. In recent years, stem cell-based therapies and stem cell-derived agents have emerged as promising approaches for periodontal regeneration. This study aims to evaluate the potential role of stem cells in periodontal regeneration and review recent clinical findings in the field. A literature review was conducted using randomized controlledtrials, systematic reviews, and meta-analyses published in the last decade. Mesenchymal stem cells derived from bone marrow (BMSCs), adipose tissue (ADSCs), and dental pulp (DPSCs) were found to have regenerative potential in periodontal applications. Additionally, exosomes, microRNAs, growth factors, and genetically modified stem cells have demonstrated supportive roles in tissue regeneration. Carrier systems such as hydrogels have been shown to enhance the delivery, adherence, and differentiation of stem cells in the target tissues. Clinical studies reported that stem cell-based therapies improved clinical attachment levels, reduced probing depths, and enhanced bone fill. However, variability in methodology and heterogeneity among studies limit the ability to generalize the outcomes. In conclusion, stem cells and stem cell-derived agents show significant potential in periodontal therapy. While current evidence is encouraging, further clinical studies are necessary to confirm their efficacy and safety in routine clinical use.
