Evaluation of the frequency of falls and risk factors in psoriatic arthritis and rheumatoid arthritis

Özet
To determine the frequency of falls in patients with rheumatoid arthritis (RA) and psoriatic arthritis (PsA), investigate the risk factors for falls, and evaluate the relationship between falls and disease activity. This is a cross-sectional study, in which participants were included over a 12-month period. 100 patients with RA and 80 patients with PsA were evaluated. The relationship between the presence of fall history and disease activity scores and balance tests was analysed. The patients' fall risk was assessed through dynamic tests, including the Tandem Stance Test (TST), Tandem Walk Test (TWT), Timed Up and Go (TUG) Test, 5 times Sit-to-Stand Test (5XSST), and Tinetti Performance-Oriented Mobility Assessment (POMA). 33% of RA patients and 46.3% of PsA patients reported a history of falls. A higher frequency of falls (>2) was observed in patients with PsA (p=0.02). In the fall tests of PsA patients, the POMA was found to be significantly lower (medium risk), and the number of patients with TUG >12 seconds was found to be higher in patients with falls (p value 0.012 and 0.046 respectively). Patients who exhibited a fear of falling demonstrated a heightened degree of impairment in fall tests. Falls were associated with disease duration and HAQ score. PsA has been demonstrated to have a higher prevalence of falls in comparison with RA. PsA patients should be subjected to a more thorough questioning process to identify potential cases of falls.
