Aim:This study aimed to evaluate the factors influencing the decision to choose or not choose thoracic surgery as a medical specialty among fifth- and sixth-year medical students. Methods: This cross-sectional study was conducted at a single center between January and July 2024. A total of 250 fifth- and sixth-year medical students who voluntarily agreed to participate were included. Data were collected through an online, structured questionnaire assessing sociodemographic characteristics and factors related to the preference for thoracic surgery as a specialty. Descriptive statistics and chi-square tests were used for data analysis, and a p value <0.05 was considered statistically significant. Results: Of the participants, 39.6% reported considering thoracic surgery as a future specialty. The most influential factors for choosing thoracic surgery were its specific field of practice (84.8%) and the perception of high professional satisfaction (83.8%). Interest in thoracic surgery, plans for an academic career, and perceived professional satisfaction were significantly associated with specialty preference (p<0.001). Although intensive and demanding on-call duties, malpractice risk, and exposure to verbal or physical violence were the most frequently reported reasons for not choosing thoracic surgery, these factors did not show a statistically significant association with specialty preference. Conclusion: Early exposure to thoracic surgery during medical education, increased clinical rotation opportunities, and the development of structured mentorship programs may positively influence medical students’ specialty choices and contribute to addressing future workforce needs in thoracic surgery.
