Özet

Objectives: Endoperiodontal lesions refer to lesions characterized by the presence of inflammatory products in both the periodontium and the pulp. In the interaction between periodontal and pulpal structures, certain anatomical features play a significant role. The combination of endodontic and periodontal treatments plays an important role in the healing of extensive periapical lesions.

Case Report: A 14-year-old, systemically healthy female patient , presented to our clinic with complaints of pain and swelling in the lower right mandibular molar region. Upon evaluation of radiographs, a large periapical lesion involving teeth numbers 45, 46, and 47 was identified. Given the possibility that the lesion could have both endodontic and periodontal origins. It was initially decided to perform endodontic treatment, followed by periodontal surgery.

During the clinical examination, pulp sensitivity tests of teeth numbers 45 and 47 were positive. Retreatment was planned for tooth number 46. For tooth 46, which had already undergone root canal treatment, flap surgery was planned. Upon raising the flap, advanced bone loss was observed on the buccal aspect of tooth 46. Granulation tissue in the area was removed and the region was irrigated with saline. To promote regeneration in the area, Platelet-Rich Fibrin obtained from the patient’s blood was placed.9 months follow-up showed complete healing of the lesion, and the patient’s clinical symptoms had resolved. Conclusions: The combined endodontic and periodontal treatment resulted in symptom resolution and significant healing of the periapical lesion. The case underscores the effectiveness of a multidisciplinary approach in managing endodontic-periodontal lesions.
