Hepatit B İnfeksiyonunda Atipik Serolojik Profillerin İncelenmesi: Tek Merkez Deneyimi, Isparta

Özet

Giriş: Hepatit B virüs (HBV) infeksiyonlarında görülebilen atipik serolojik profiller, laboratuvar sonuçlarının yorumlanmasını ve hasta yönetimini güçleştirmektedir. Bu çalışmada, Süleyman Demirel Üniversitesi Araştırma ve Uygulama Hastanesine başvuran ve HBV infeksiyonu açısından değerlendirilen hastalarda atipik serolojik profillerin araştırılması amaçlanmıştır. Materyal ve Metod: Ocak 2022-Aralık 2023 tarihleri arasında hastanemize başvuran 45.063 hastanın verileri atipik HBV serolojik profil varlığı açısından retrospektif olarak değerlendirilmiştir. Serum örneklerinde HBV belirteçleri elektrokemilüminesans immünoassay yöntemiyle (Cobas e601, Roche Diagnostics) analiz edilmiştir. Hepatit B virüs DNA varlığı, gerçek zamanlı polimeraz zincir reaksiyonu yöntemiyle (Cobas 4800, Roche Diagnostics) araştırılmıştır. Atipik profile sahip olguların demografik özellikleri ile birlikte hepatit D virüsü (HDV) antikor (anti-HDV), hepatit A virüsü (HAV) antikor (anti-HAV IgM ve anti-HAV total), hepatit C virüsü (HCV) antikor (anti-HCV) ve insan immün yetmezlik virüsü [human immunodeficiency virus (HIV)] antikor (anti-HIV), HCV RNA, HIV RNA, aspartat aminotransferaz (AST), alanin aminotransferaz (ALT), romatoid faktör (RF) ve anti-nükleer antikor (ANA) test sonuçları incelenmiştir. Bulgular: Atipik serolojik profil saptanan 520 (%1.15) hastanın 416 (%0.92)’sında izole anti-HBV core (HBc) pozitifliği, 91 (%0.20)’inde HBV yüzey antijeni (HBsAg) ve anti-HBs birlikte pozitifliği ve 13 (%0.03)’ünde HBeAg ve anti-HBe birlikte pozitifliği tespit edilmiştir. İzole anti-HBc pozitif olguların %11.7’sinde HBV DNA pozitifliği saptanırken ileri yaşlarda ve erkeklerde daha sık görüldüğü gözlenmiştir (p< 0.05). Bu olgularda anti-HCV, anti-HIV, RF ve ANA pozitifliği sırasıyla %4.2, %1.1, %20.5 ve %12.7 oranında bulunmuştur. HBsAg ve anti-HBs birlikte pozitif olguların %94.2’sinde HBV DNA, %90.6’sında anti-HBc total ve %92.5’inde anti-HBe pozitifliği tespit edilmiştir. HBeAg ve anti-HBe birlikte pozitif olguların tamamında HBV DNA pozitifliği saptanırken diğer atipik profil gruplarına göre daha yüksek AST ve ALT düzeylerine sahip oldukları görülmüştür (p< 0.05). Sonuç: Çalışmada, en sık saptanan atipik serolojik profil izole anti-HBc pozitifliği olmuştur. Hepatit B virüs infeksiyonunun seyri sırasında preanalitik/analitik hata, otoantikorlara bağlı çapraz reaksiyonlar, koinfeksiyon varlığı gibi birçok faktör atipik profillerin ortaya çıkmasına neden olabilir. Atipik profil sergileyen olgularda ileri tetkiklerin uygulanması ve yakın takip gereklidir.
Introduction: Atypical serological profiles in hepatitis B virus (HBV) infections make the interpretation of laboratory results and patient management difficult. In this study, it was aimed to investigate the atypical serological profiles in patients admitted to Suleyman Demirel University Research and Practice and evaluated for HBV infection. Materials and Methods: The data of 45.063 patients admitted to our hospital between January 2022 and December 2023 were retrospectively evaluated for the presence of atypical HBV serological profile. HBV markers in serum samples were analyzed by electrochemiluminescence immunoassay method (Cobas e601, Roche Diagnostics). The presence of HBV DNA was investigated by realtime polymerase chain reaction method (Cobas 4800, Roche Diagnostics). Demographic characteristics of cases with atypical profiles along with anti-hepatitis D virus (anti-HDV), anti-hepatitis A (anti-HAV) virus IgM and total, anti-hepatitis C virus (anti-HCV), HCV RNA, anti-human immunodeficiency virus (anti-HIV), HIV RNA, aspartate aminotransferase (AST), alanine aminotransferase (ALT), rheumatoid factor (RF) and antinuclear antibody (ANA) test results were examined. Results: Among 520 (1.15%) patients with atypical serological profile, isolated anti-hepatitis B core (HBc) positivity, concurrent hepatitis B surface antigen (HBsAg)/anti-HBs positivity, and concurrent hepatitis B envelope antigen (HBeAg)/anti-HBe positivity were detected in 416 (0.92%), 91 (0.20%), and 13 (0.03%) cases, respectively. HBV DNA positivity was found in 11.7% of isolated anti-HBc positive cases, and the profile was observed to be more common in older ages and in males (p< 0.05). In these cases, anti-HCV, anti-HIV, RF and ANA positivity were found to be 4.2%, 1.1%, 20.5% and 12.7%, respectively. HBV DNA, anti-HBc total, and anti-HBe positivities were detected as 94.2%, 90.6%, and 92.5%, respectively, in patients with concurrent HBsAg/anti-HBs positivity. HBV DNA positivity was detected in all concurrent HBeAg/anti-HBe positive cases, and it was determined that they had higher AST and ALT levels compared to other atypical profile groups (p< 0.05). Conclusion: In our study, the most frequently detected atypical serological profile was isolated anti-HBc positivity. During the course of HBV infection, many factors such as preanalytical/analytical errors, cross-reactions due to autoantibodies, and the presence of coinfection may cause atypical profiles. In cases exhibiting atypical profiles, further examinations and close follow-up are necessary.
